Personal injury collision occurred 9-23-15 at approximately 1720 hours on US64 0.5 miles east of County Road 36800, approximately 3 miles south of Cleveland, OK in Pawnee County.

Passenger- 1: Mariah N Bibbs, black female, age 21 of Hominy, OK. Transported by AirEvac to St Francis, Tulsa. Treated and released.
Passenger- 2: Name withheld, juvenile female, age 1 of Hominy, OK. Transported by Mercy EMS to St. Francis, Tulsa. Treated and released.
Vehicle- 2: 1956 Volkswagen driven by Robert G Burk, white male, age 64 of Mannford, OK. Transported by Mercy EMS to St. Johns, Tulsa. Admitted in stable condition with head, trunk external, trunk internal, and leg injuries.
Vehicle- 3: 2004 Acura driven by Jennifer K Holder, white female, age 42, of Cleveland, OK. No injuries.
What Happened- Vehicle 1 was eastbound on US64, vehicle 2 was westbound on US64. Vehicle 1 crossed the center line and struck vehicle 2. Vehicle 1 then departed the roadway to the left, coming to a rest in a ditch. Vehicle 2 departed the roadway to the right coming to a rest on the shoulder. Vehicle 3 was following behind vehicle 2 and was struck by debris from the initial collision. Unit 3 came to a controlled rest on the shoulder.
Condition of Driver- Apparently normal
Cause of Collision- Inattention/ use of cell phone.
